At AIRnyc, our highly trained team of Community Health Workers (CHWs) meets people where they live to improve health, connect families to critical resources, and build health equity. Our community-based organization has been transforming the lives of vulnerable New Yorkers for more than two decades through expert care coordination and coaching interventions to guide individuals and families on a healthier path. AIRnycs CHWs integrate with clinical care management teams to support patients living with chronic conditions such as asthma, COPD, congestive heart failure, and diabetes, or who are in need of maternity care support. CHWs also help to disseminate important public health information, and provide social needs screening and navigation to assist people who are struggling with housing instability, unemployment, food insecurity, and more.
