Community action program for children (capc) and canada prenatal nutrition program (mothercare): promotes the healthy development of children (0 to 6 years) living with parents who are young or have low-incomes.,ontario autism program: provides case management support to children with a diagnosis of autism spectrum disorder,and their families.,transitional age youth coordination (tay): supports youth transitioning from child welfare to adult services. assists with the development of a transition plan and coordinates services as youth moves into adulthood.,family resource services: works in partnership with families who have a school-aged child with a developmental disability and/or a physical disability to identify their strengths and needs and to access necessary and available resources.,fetal alcohol spectrum disorder (fasd) initiative and key services: this initiative is to provide coordination and leadership in building a comprehensive approach to prevention,diagnosis and interventions for individuals and families in simcoe county. the key services coordinator provides support to families.,wraparound program: assists families for whom traditional services have not been effective to achieve positive changes that the family deem important. the wraparound facilitator meets and learns about the family’s strengths,what they like or don’t like,what they need and what they want for the future. they will help others to understand the needs of the family and build a team of family friends and professionals to follow a plan for the family.,intensive service coordination: provides information,consultation,and community case conferencing for children/youth and their families who have complex needs requiring special attention and creative service planning.,regional complex care services: works with families who have children 0-18 years of age. children/families are referred to our service through intensive service coordination services. intensive service coordination will make sure that all other community options for services have been considered first.,adult protective service worker: assists adults with developmental disabilities living within their community setting,with families/care giver or independently to develop a life plan of their choice,to access the services and supports they need to meet their life goals.,adult complex care: provides case management and funding to support youth over 18 years of age with complex needs leaving the children’s complex care system in simcoe county.,crisis response network coordination: supports 18 and over,who have a dual diagnosis (developmental disability and mental health problems and/or challenging behaviours) who are experiencing a situational crisis.,passport: improves the quality of participation in the community for adults with a developmental disability. passport funding can be used to purchase a day program,to hire a worker to assist the person to participate in community activities,or a combination of both.,respite care services (children and adult): a family support service that provides temporary relief from the physical and emotional demands involved in caring for a family member who has a disability. respite creates greater opportunities for all family members to live actively and participate in community activities and allows parents/ guardians time for themselves. the service can be arranged in the family’s home,in the care provider’s home,or in a community setting.,and service planning coordination: working with families to support the development of a coordinated and integrated service plan that considers family needs and strengths. a family-centred approach,guiding and supporting family and child to identify and reach goals.
