Through an in-home assessment, we listen to your needs and develop an individualized care plan that links you to a variety of home and community based services and resources. Each office has an individual who can assist you in locating the optimal services for you and your family. In six counties in Illinois, CCSI Case Coordination, LLC. operates a Care Coordination Unit who is designated by the Illinois Department on Aging. The Care Coordination Unit helps older adults and their caregivers in finding needed services and resources in the community to maintain their independence.
