CHC is a Joint Commission accredited home care company that services patients across the state of Massachusetts. Our vision supports giving exceptional care to patients while being mindful of not creating dependence on services, but to teach independence and self-care in a way that allows the patient to live and remain safe in our community. We are a skilled in-home nursing agency that offers nursing, therapies and home health aide to help individuals live independently in the community. Eighty percent of CHCs patients have mental health and addiction diagnoses, as well as standard medical diagnoses. We are the only home care company in the state who prepares all of its qualified nurses to sit for the examination to become board certified psychiatric nurses.
