We provide honest and professional care, bringing together home and community support services to focus on improving the welfare of people in need. OUR STORY Community Health and Wellness began 10 years ago out of a one bedroom apartment, and it has grown to be a thriving Healthcare Company that provides specialized services to individuals with impairments, disabilities, and mental health diagnoses. Community Health and Wellness provides private programming--therapies and classes as well as services to clients on three state Medicaid waiver programs: Youth Empowerment Services (YES) Waiver, Home and Community Services (HCS) Waiver, and Community Living Assistance and Support Services (CLASS) Waiver.
