Constance Health and Social Care is a provision which gives support and accommodation for people who have been diagnosed with complex needs such as learning disabilities and/or Autism or mental ill health. We have created an environment that provides support and care based on individual assessed needs. Our goal is to help individuals live as full a life as best as possible. A fulfilled lifestyle and achievement of their personal best. We work closely with specialist housing partners and care organisations, keeping the well-being of our service users at the very centre of our objectives. Our service is open to all individuals, regardless of their religious, cultural, ethnic and sexuality. We are proud of our diversity ethos. We are happy to accept the following: - Long term - Short term - Crisis Support We will provide details of our referral and assessment process upon request. Unfortunately we are unable to accept those with a history or arson or fire setting
