Health Horizons International (HHI) is a non-profit organization dedicated to improving community health and expanding access to quality primary health care in the Dominican Republic. Our model is based on three pillars: - Developing Community Leaders Our Community Health Workers Program trains and supports local individuals as partners in primary care delivery, focusing on chronic disease management and prevention. - Strengthening the Health System We collaborate with health sector leaders to build capacity for achieving public health priorities, including addressing the growing challenges of chronic disease through a primary care lens. - Improving Access to Quality Care Our Chronic Care Program provides medication and counseling to patients in rural communities by creating a network of physicians and CHWs who provide continuity of care.
