Horizon Care Services provides medical and non medical home care services for those who are in need of private duty nursing or personal care services. Our focus at Horizon Care Services has always been for seniors to stay at home safely for as long as possible. However when health problems are a concern, illnesses develop, or accidents happen, an elderly person may need additional home care services or care in a facility. Our goal as care managers is to coordinate needed healthcare and community services to meet the short-term and long-term care of our clients.
