Our mission: to reduce the burden of these diseases and their associated risk factors at both individual and societal levels. Were committed to promoting health systems that are sustainable and equitable for all. How do we achieve this? By promoting best practices: Identifying strategies across the patient journey in six priority areas, from health literacy to screening high-risk populations, integrated care pathways, self-management support and workforce participation.
