Mahitahi Hauora

Mahitahi Hauora is Northlands largest Primary Health Organisation. A significant proportion of our enrolled patients are Maori, many are living in rural areas in high levels of deprivation and have complex health needs. Our vision is an equitable healthcare system that sustains self-determination and improves health outcomes for all whanau living in Te Tai Tokerau. To achieve this, we work collaboratively with our general practices, communities, iwi, and various other organisations to delivery health and wellbeing services. In collaboration with our service providers, we are developing and implementing an integrated delivery model between health and social services providers to deliver care that is evidence-based, person-centred, and able to address the wider determinants of health and wellbeing.
