Optima Health Community Care

Optima Health has been selected to contract with the Virginia Department of Medical Assistance Services (DMAS) to provide Medicaid managed long term services and supports to Virginians with complex health issues. Eligible individuals will be enrolled in the new statewide Commonwealth Coordinated Care Plus (CCC Plus) Program. CCC Plus will build upon the foundation of Virginias current Dual Demonstration Program: Commonwealth Coordinated Care (CCC). CCC Plus will provide services to approximately 214,000 individuals including dual eligible (Medicaid and Medicare A and/or B) members, individuals receiving long term services and supports through a nursing facility or a home and community-based services waiver, as well as Virginias aged, blind, and disabled populations.
