Pathway is the UKs leading Homeless and Inclusion Health charity, working with the NHS and other partners to create improved models of care for people experiencing homelessness and other excluded groups, including Gypsy, Roma, Traveller communities, people who do sex work, and vulnerable migrants. In the last three years, our Pathway Partnership Programme has helped 9 NHS Trusts in England to create in-reach Pathway teams of doctors, nurses, social care professionals and housing specialists who support over 3,500 patients experiencing homelessness every year.
