PPO Serve is a Value Based Care healthcare management service company, which promotes the formation of Multi-Disciplinary Teams structured to deliver patient centred clinical care. Our clinical products work at high volume and deliver good quality at low cost. Contracts are based on risk adjusted global fees, with a value linked portion that drives continual improving patient outcomes. We work closely with clinicians and medical schemes to accelerate the transition to value-based care models and help clinicians to manage the long-term health of their patients. We help clinicians deliver Value Based Care by creating products delivered by collaborative multidisciplinary teams, that puts them in control of the care of patients and fairly remunerates them for the value they produce.
