Prescribing Care Direct (PCD) aims to develop and deliver a high quality medicines optimisation and management service to GP surgeries nationwide. At PCD, we provide a bespoke medicines management service. Through our highly skilled and experienced pharmacist support team, our practice pharmacist model supports surgeries with a cost effective resolution to manage medicines related activities. PCD has recognised an increasing need for the deployment of practice pharmacist services. We provide a range of primary care medicines optimisation services, including care home and discharge support services. Our pharmacists are now also supporting practices with patient facing activities such as multiple morbidity and medication review clinics.
