Rely Health provides you with the infrastructure to design and deploy EHR-Integrated Care Navigation program(s) within 4 weeks for any hospital or health plan use-case. We recruit, train, and staff local Patient Care Navigators that can serve and in-person or remote liaisons to drive patient care. A technology suite is used to design modern digital pathways for your health system / health plan. Patients or members never have to download an app or set a password; all engagement is delivered through modern native channels (SMS-Text, Email, Phone Call). Patient Care Navigators have access to a Patient Relationship Manager (proprietary software) that seamlessly integrates with any EHR to drive digital journeys, manage task queues, and facilitate follow-up actions such as automated appointment reminders.
