Salford Primary Care Together

We are here to put general practice at the heart of joined up care in Salford We have outstanding health and social care services in Salford but local people have told us they would like to see people involved in their care working more closely together. All the citys GP practices have agreed to work together in five neighbourhoods Broughton; Eccles and Irlam; Little Hulton and Walkden; Ordsall and Claremont and Swinton. We are responsible for driving this collaboration forward and supporting the practices to pool their services, staff and resources so that all GP practices in the neighbourhood are strengthened and improved. This also means connecting general practice with other professionals and support in local neighbourhoods, such as district nurses, social workers, mental health workers, voluntary organisations and local community groups.
