Our Purpose is to keep Tasmanians out of hospital and enjoying a healthy life at home and in their community. Our Vision is to partner with our clients to improve personal engagement in their own healthcare, to improve their health literacy and to ultimately improve their health outcomes and keep them out of hospital where possible and appropriate. To do this we wish to provide our clients with access to their own personal health record and for those living with a complex condition, to provide hands on home based support to help coordinate their care, improve their health literacy and to identify potential risk factors that may lead to an avoidable hospital episode.
