Vitalyst Health Foundation has three goals: 1) Collaborative Networks working to improve the conditions that impact health. 2) Strong Community Partners equipped to advance health equity. 3) Informed Decision-Makers prioritizing community health and well-being. Our goals span across four priorities: Civic Health - Supporting long-term efforts that improve active community participation. Food Systems Strengthening food systems across rural and urban areas in Arizona. Health Care Integration Supporting the integration of physical, mental and social determinants of health. Housing Supporting statewide housing initiatives along the spectrum of housing. Vitalyst Health Foundation has been an Arizona nonprofit corporation since 1907. For most of its existence, the nonprofit operated one or more hospitals in the Phoenix metropolitan area, including St. Lukes Medical Center and St. Lukes Behavioral Health Center in Phoenix, Arizona and Tempe St. Lukes Hospital in Tempe, Arizona. ...
